A Lost Case in the System: The MDT Pathway Failure
But here's where it gets controversial. After James' hospitalization in May 2024, doctors arranged a multi-disciplinary team meeting (MDT), where his cardiologist advocated for a surgical referral. However, a surgeon declined, and a subsequent meeting in July failed to include James' case on the list. Area Coroner for Manchester North, Catherine McKenna, stated, "His case was lost in the MDT pathway."
Dr. Palmer believed surgery should have taken place in the summer of 2024, but James' treatment was delayed until the New Year, by which time he was critically ill.
The Tragic Outcome: A Preventable Death?
When James finally underwent surgery in March 2025, it was too late. The surgeon realized the extent of James' illness only after seeing the damage to his heart muscle. A week later, James succumbed to multiple organ failure caused by cardiac failure, triggered by constrictive pericarditis.
Dr. Palmer explained, "The longer you leave a condition like this, the more the heart is affected." He believes that if James had received treatment earlier, his chances of survival would have been significantly higher.

McKenna recorded James' death as a natural cause, stating that pericarditis is a rare but naturally occurring condition. This conclusion left James' family shocked and disappointed.
"We are shocked it's gone down as a natural death," Emma said. "We don't want any family to go through this. If he had been treated earlier, things would have been different. James was failed by the Northern Care Alliance."
Dr. Rafik Bedair, Chief Medical Officer at the NCA, offered condolences but acknowledged that the care James received fell short of expectations. The trust has since taken steps to improve and prioritize urgent cases.
